Collateral History Forms
ADHD Forms
For Child ADHD assessments, a SNAP-IV Form and a Child Collateral History Form will need to be completed by at least one teacher prior to the end of the assessment pathway.
If this is not possible then please contact our team for advice.
For Adult ADHD assessments, a SNAP-IV Form and an Adult Collateral History Form will need to be completed by a relative or close friend prior to the end of the assessment pathway.

Autism Forms
For Child Autism assessments, a Child Collateral History Form will need to be completed by at least one teacher prior to the end of the assessment pathway.
If this is not possible then please contact our team for advice.
For Adult Autism assessments, an Adult Collateral History Form will need to be completed by a relative or close friend prior to the end of the assessment pathway.
